At our first meeting, we'll asses needs, services, and options that best support the situation. You will leave this meeting with a full understanding of next steps and appointments scheduled.

Second Meeting

The second meeting we take at the home of the patient. We evaluate the living space and make safety outfitting recommendations based on industry standards. We also sit and meet with the patient if they were not at the first meeting. This is also the visit where we get your information to build out your account in our EHR system, sign paperwork, obtain payment information and schedule the first start of care appointment.

If ant outfitting needs to be orchestrated, we can also coordinate that at this visit. est 2.5 hours

Third Meeting

This is our start of care meeting. This will actually be an assessment preformed by an RN. They will assess the patient for key motor functions, capabilities, and needs. They will then work with the family to schedule the next weeks services. We will retain this assessment and utilize it as a base measure moving forward. est 3 hours

Initial Intake Meeting

Decide on your account type during our initial intake meeting. No matter what you choose, you are choosing the highest level of care. The only difference is the level of concierge you want in your service.

We will go over our services and options and create a plan perfect for you. est 1 hour

Following That...

Depending on your scheduling preference, your care manager will call you weekly to set up services or wait for you to call us. On top of that, you will receive bi annual medical assessments and family meetings.

If you prefer to reach out to us, if we don't hear from you, we will reach out once a month to make sure your needs are met and are happy, other than that, we won't be a bother
